Case study: Enfield South West PCN - Hypertension patients

The challenge 

Our partner, Enfield South West PCN, as part of North Central London ICB, tasked Suvera with fully managing their hypertension register and to achieve normotensive ranges for HYP 003/007 indicators. The overall PCN list size is 50,000.

What we did

We deployed our virtual clinic service, onboarding their hypertension patients onto the Suvera care pathway, launching the process of monitoring and caring for their condition remotely. This included empowering patients to proactively submit key health readings via the patient web app, to be monitored in real-time by Suvera’s clinicians.


We completed the financial year with an average of 83% target achievement across all practices, surpassing the target of 77%. Importantly, as the chart below shows, we achieved significant progression across all practices within the PCN.
